How do I access my health records or Health Benefits Claims Records?

You or your authorized representative can request access to or copies of your health record by completing the Authorization for Release of Information Form and submitting it in person, by fax, or by mail to the location where you received care or to Information Management Services. They will assist you in accessing the records you wish to see and are entitled to receive.

Is there a fee?

There are generally no fees, but you may be charged for actual services related to the processing of your request including copying of records, courier fees, special delivery or other special processing services.

When can I expect a response?

You will receive a response within 30 business days from the date we receive your written request. If we need to consult with others or if we have a large volume of records to review, the time may be extended, as permitted under the BC Personal Information Protection Act. If there is a delay, we will notify you in writing of the reason for the extension and the new due date.

If you are not satisfied with the response you receive, contact Information Privacy Office or the BC Office of the Information and Privacy Commissioner.
