This tool is a way for your voice to be heard if you or someone you know has come across an accessibility barrier. An accessibility barrier is anything that prevents the full and equal participation of a person with a disability in society. Barriers can be caused by environments, attitudes, practices, policies, information, communications, or technologies.
Historical and ongoing colonial practices continue to affect many First Nations with disabilities. As we learn more about accessibility and what it means for First Nations, we hope that in partnership we can remove stigma and barriers for FNHA employees and the many communities that FNHA works with. Please follow the below instructions to provide feedback.

Your participation in this feedback tool is voluntary. Information provided by you will be used to enable the FNHA to better understand the perspectives of those individuals who may have faced barriers to accessibility at the workplace and to evaluate FNHA's Accessibility Plan.
Your information will be reviewed by FNHA Accessibility Program personnel. Your responses will be de-identified and summarized and shared with FNHA's Executive Team. Identifiable data will be deleted once the data has been summarized. Should you have any questions about the collection, use and disclosure of your personal information, including to withdraw your consent, please contact accessibility@FNHA.ca.

What to expect after submitting. The FNHA Accessibility Lead will:
- send an acknowledgement of your feedback and gather any additional information required;
- assess and determine the potential actions in response to your feedback; and,
- with your consent, pass on your feedback to the relevant individual, team, or service you have an inquiry on.
